Policies:

New patients, please arrive 15 minutes before your scheduled consultation to complete a patient profile. Note: There is only ONE complimentary consultation. 

Cancellation Policy:

Your scheduled appointment is reserved exclusively for you. Should you need to cancel or reschedule your appointment, please notify us 24 hours in advance to avoid a charge. If an appointment is rescheduled or cancelled in less than 24 hours, there will be a $50 charge for your missed visit. (i.e. If you have an appointment scheduled for 1pm the following day, calling to notify the office at 3pm does not qualify as ample notice).
